"My recommendation on the prescription for window tint would be to formalize it. Have the doctor provide you with a type-written (clear and legible) "prescription" for the tint (a specific tint percentage would help if possible) on a letter head from his office, signed and dated by him, including his contact information (i.e. address, phone, etc.). Some people are writing up their own "notes" and are just not good enough. We need to be able to verify the information."
