It doesn't look fake to me. I've seen pics of a guy missing that whole lower part of his face due to a genetic disease, I wouldn't be surprised if that was real. If it's not, the hospital setting in the backround is a nice touch.

I would think his eyes would be swelled shut if it was real. I got hit in the head once and it was nothing like that. Within 15 min I couldn't see anything. Both of my eyes had swollen shut and I was hit on the right side of my head (with an aluminum baseball bat).
